Youth In Transition

This program provides case management services to youth ages 16 to 24 who have a confirmed diagnosis of mental illness or are displaying symptoms of mental illness. Services are designed to connect youth to needed psychiatric services and to promote the skills necessary for youth to live independently in the community while coping with mental illness.
