Program Description
Rapid Response Outreach Services is a program that provides short-term brokerage case management services. The focus of these case managers is to engage, assess and assist consumers to address to their issues. This service operates within a time period of 1-6 months depending on client need. This service includes assisting consumers to secure housing and accessing all community resources. This program has three components that service:
- Case management support for individuals being diverted from a hospital stay. Referrals received from the Mental Health Assessment Team of the Thunder Bay Regional Health Sciences Centre and the Short-Term Urgent Care Program of the St. Joseph’s Care Group Health Centre.
- Case Management support for individuals being discharged from the Thunder Bay Corrections Centre.
Who can access the Services?
Any person who is homeless or at risk and is experiencing mental health difficulties and or addictions wanting to get help.
We can provide the following services:
- Short-term brokerage case management services.
- Assistance in accessing mental health and/or psychiatric services.
- Assistance in accessing drug and alcohol treatment.
- The staff will utilize a range of intervention strategies from a harm reduction approach to an abstinence based model in efforts to assist in addressing substance use addictions.
- Help to integrate into the community, advocating with informal and formal community resources. Referrals to community agencies, where appropriate.
- Assistance in accessing income support, i.e. Ontario Works, O.D.S.P.
- Assistance in obtaining and maintaining housing in the community.
Admissions Criteria:
- Any person who has a desire to address either their mental health or addictions' issues and are connected to services mentioned above.
Exclusion may be deemed necessary due to one or a combination of the following:
- Unwilling to participate
- Repetitive history of violent or abusive behaviour
