Applying for Services

Referral Process

Individuals may self-refer at the Wilson Street office. 

All individuals interested in supportive housing and case management are required to use the Mental Health Case Management Intake "Request for Services" form to apply for services.

 

What is Mental Health Case Management Intake:  

It is the central location that receives the Request for Services forms and directs your referral to a participating program that best meets your needs. This process ensures people are accessing the right care at the right place at the right time.
